Primary Responsibilities:


  • General Inquiry, Outreach and Education - (While following all PHI and HIPAA guidelines)

    • Provide program information and education to members utilizing established protocols, procedures, and educational methodologies via the telephone in an accurate and courteous manner.

    • Complete a Health Risk questionnaire with enrollees into the program.

    • Encourage members to appropriately utilize services in an effort to improve the health and well being of all members. This might include education about the importance of preventive care or specific information about the member' condition or disease state.

    • Provide ongoing education for all members based on individual needs identified through questions or conversations, direction from case managers or system identified needs.

    • Identify barriers to behavior change and work with member to assess willingness to change.

    • Utilize persuasion skills to solicit change and/or increase member desire to change behavior. (Note - Activities may occur while taking inbound calls or making outbound calls to identified members.)



  • Problem Resolution and Intervention

    • Develop and implement member-specific interventions when necessary. This may include calls to provider offices, transportation services or case management staff.

    • Determine when an issue or complaint requires higher-level review or referral to a case manager or other entity involved in the program and input or provide the appropriate information to ensure prompt and accurate escalation of the issue. These issues can be program related or health related.

    • Research claims data to determine if program member needs follow up education calls.



  • Referrals to Other Resources

    • Identify when a member needs to speak to a case manager or other entity participating in the program. Clearly explain the reason for the referral and provide the appropriate number for the member to call.

    • Assist members in identifying other program or community-based resources.

    • Develop knowledge of and linkages to other community-based organizations to improve access to services.



  • Documentation

    • Document all contacts with members, providers, case managers or other resources in the database completely and accurately to ensure accurate tracking, statistics and member history.

    • Provide documentation of perceived system or process issues to Supervisor to assist in quality monitoring and improvement.



  • Other Duties, as Appropriate and Assigned

    • Assist with projects and/or assignments designated by the Supervisor or Manager.

    • Provide input into the development of policies and procedures that affect the program or department. Give input into improving member education strategies and materials.

    • Actively participate in departmental quality improvement monitoring and other activities to promote the delivery of high quality services to all customers.

    • Assist Supervisor with orientation/training of new employees as assigned.

    • Assist with special projects or other duties as assigned.

Skills / Requirements

Requirements:




  • Bachelor's Degree in social work, health education, healthcare administration, psychology, or sociology with 2 years of experience within a healthcare, education or community based service field required.



  • If you have a Bachelor's Degree but not within one of these majors then 4 or more years of work experience within healthcare, education, or community based service field required.



  • 2+ years of customer service experience in a professional environment required.



  • 2+ years experience in a healthcare education or community based field is required



  • Proficiency with MS Word and Excel required.


Assets:

    Previous experience working with Medicaid or at risk populations preferred.
    Experience working with low income populations preferred.
    Managed Care experience preferred.
    Ability to type 40 WPM helpful.
    Ability to use Internet searches and navigation through databases preferred.
