JOB SUMMARY:
The primary purpose of this position is to administer medications and treatments under the supervision of a licensed nurse and to assist with the daily care of residents in accordance with facility policies and procedures and as may be directed by your supervisor.
QUALIFICATIONS:
1. Must be currently certified as a CNA/QMA
2. Must be able to read, write, speak and understand the English
language
3. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
4. Must possess the willingness to work harmoniously with other department personnel.
5. Must be willing to seek out new methods and principles and be
willing to incorporate them into existing training practices.
6. Must posses the ability to plan and develop new programs.
7. Must have patience, tact, and a cheerful disposition and enthusiasm.
8. Must be able to relate information concerning resident's condition.
PHYSICAL REQUIREMENTS:
1. Must be able to move intermittently throughout the work day.
2. Must be able to cope with the mental and emotional stress of the
position.
3. Must be able to see and hear or use prosthetics that will enable these senses to function adequately to assure that the requirements of the position can be fully met.
4. Must be able to function independently and have flexibility, personal integrity and the ability to work effectively with residents, personnel, support agencies and outside agencies.
5. Must be in good general health and demonstrate emotional stability.
6. Must be able to lift, push, pull and move a minimum of 50 pounds.
POTENTIAL EXPOSURE TO BLOODBORNE PATHOGENS
This position is a Category I position. Tasks may involve exposure to blood, body fluid or tissue, requiring the use of personal protective equipment. Gloves should be worn for any contact with blood or body fluids/tissue. Gowns and masks or eye coverings may be needed if it is likely that soiling of clothing or splattering of blood may occur.
POTENTIAL EXPOSURE TO WORKPLACE VIOLENCE: Category 4 - HIGH RISK
Tasks that have a high possibility of coming into contact with a violent person or situation but employment may require performing unplanned category 4 tasks. The normal work routine involves a very high possibility of coming into contact with a violent person or situation at least once per shift, if not more often however, contact or potential contact at some time may be required as a condition of employment.
ENVIRONMENTAL CONDITIONS:
1. Sits, stands, lifts, and moves intermittently during working hours.
2. Works beyond normal working hours and on weekends and holidays
when necessary, as well as in other positions as needed.
3. May work flexible hours.
4. Is subject to frequent interruptions.
5. Attends and participates in continuing educational programs.
ACCOUNTABILITY: Is accountable to the Licensed Nurse.
ESSENTIAL JOB FUNCTIONS of: Qualified Medication Assistant
1. To know and follow all rules of state/federal guidelines, Provena Sacred Heart Home policies, Medicare, and Medicaid regulation, state and federal rules. Any and all violations of the above will be reported to your supervisor.
2. Receives report when arriving. Shares all information concerning resident care necessary to see that continuity of care is provided.
3. Protects and maintains residents confidentiality by giving information only to those with a right to know. Resident information will only be shared in private settings.
4. Works in role of CNA whenever and wherever needed and does so
graciously.
5. Answers resident call lights and provides direct hands on care.
6. Administers and documents medication to resident as per program guidelines and notes resident responses. Consults with licensed nurse prior to administration of PRN medication and documents responses.
7. Completes topical treatments (noninvasive) as ordered by physician and notes resident responses.
8. Notifies licensed nurse of resident condition changes and seeks appropriate intervention for care.
9. At all times, deals professionally with residents, families, staff and physicians. Professionalism is evidenced in language (no cursing or slang use), dress (follows established dress code), behavior (friendly and polite, but resident focused) and attitude (positive and cooperative).
10. Adheres and maintains to the attendance policy.
11. Attends 16 hours of inservices per year to remain current with trends in nursing, new medications, new policies, etc.
12. Attends staff meetings and serves on other committees as assigned.
13. Represents the nursing department in a positive light to resident, families, physicians, and another departments. Takes concerns through proper chain of command.
14. Participates in all survey processes.
15. Upholds and adheres to Provena Sacred Heart Home mission, vision,
and values and applies these and Service Excellence principles to
daily work activity.
16. Adapts to and accepts changes graciously within the department and long term care industry.
17. Demonstrates a positive attitude, encourages team work, reinforces
staff effort and attitude in a positive way.
* 18. Performs all other duties as assigned to ensure the health, safety and
well being of the residents and to ensure the efficient running of the
health facility.
GENERAL REQUIREMENTS:
1. Maintain a neat, well-groomed appearance and adhere to the dress
code of the position.
2. Complies with Infection Control guidelines and follows standard
precautions and isolation procedures per written polity of the facility.
3. This position may be flexible in number of hours worked per week.
First shift will work 6:30 a.m. - 3:00 p.m.; second shift will work
2:30 p.m. - 11:00 p.m.; third shift will work 10:30 p.m. - 7:00 p.m. unless otherwise arranged with your supervisor. This job includes every other weekend and every other holiday unless status is on call or in house pool.
