Eskenazi Medical Group is seeking a Spanish Bilingual Nurse Practitoner for primary care in Indianapolis. Eskenazi Medical Group is a large primary care group whose mission is "To be the nation's leading primary care provider serving all patients regardless of ability to pay or condition of health while advancing medicine through teaching and research." Eskenazi Medical Group is affiliated with Eskenazi Health and provides primary care outpatient services in several community health centers within Marion County in addition to providing inpatient services at Eskenazi Hospital.

Under the supervision of an attending physician of Eskenazi Medical Group, plans, directs, and evaluates patient's health care at Eskenazi Medical Group venues. Examines and treats chronic and acute episodic illnesses, minor injuries, accidents, and other injuries/illnesses. Performs or orders selected laboratory testing, referrals to physicians as needed, writes prescriptions when having appropriate approved prescriptive authority, and performs office procedures within the scope of expertise protocols and available equipment. Works with primary care physicians in the care and treatment of patients.

1. Provides initial and ongoing assessment of patients: medical, physical and psychosocial status including:
a. Obtaining relevant health and medical history from patients and/or caregivers,
b. Performing physical exam appropriate for age and history, including comprehensive functional assessment when clinically indicated,
c. Conducting preventive screening procedures based on age and history,
d. Identifying medical and health risks and needs,
e. Updating and recording changes in health status,
f. Ordering, conducting and interpreting appropriate diagnostic tests,
g. Identifying the needs of the individual family or community as a result of the evaluation of the collected data,
h. Formulating the appropriate differential diagnosis based on history, physical exam, clinical findings and diagnostic test findings,
i. Developing care plan and communicating plan to patient, their care giver, or family,
j. Identifying non-pharmacologic interventions and prescribing non-pharmacologic therapies; identifying pharmacologic interventions and prescribing pharmacologic therapies when having approved prescriptive authority from the State of Indiana,
k. Providing relevant patient education,
l. Making appropriate referrals to physicians and other health professionals and community agencies,
m. Determining the effectiveness of care plans for documentation of patient outcomes and reassessing and modifying the plan that is necessary to achieve medical and health goals, communicates findings and plan of care to patient, their care giver or family.
n. Participates in quality improvement review on a periodic basis, including systematic review of records and treatment plans, helps disseminate findings and implement plans for improvement.
o. Serves as educational resource for other EMG staff.
p. Serves as a member of various EMG committees dealing with healthcare.
q. Completes and maintains written records in a timely fashion and in compliance with NCQA and Eskenazi Medical Group standards.

 Performs other related duties as assigned.
 Collaborates with staff of affiliated institutional facilities to provide appropriate care for Eskenazi Medical Group patients.
 Confers with the patient's primary care physician as needed.
 Maintains competency in area of expertise
 Assists in planning, implementing and evaluating new programs of care.
 May perform clinical services during "on-call" hours.

An individual in this position may be exposed to:
 Adverse indoor and outdoor environmental conditions
 Bloodborne pathogens
 Physical hazards (e.g., needle pricks, etc.)
 Chemical hazards

Must have MSN from accredited school; be board certified and must be able to obtain Indiana prescriptive authority.

Additional Hours may be needed: Saturday 9AM – 6PM and Monday, Tuesday, and Wednesday 4PM – 8PM , or 11AM to 8PM.
